In her roles as Staff and Head Nurse at Sibley and Suburban hospitals as well as Montefiore Hospital in New York City, Susan recognized that the patients she encountered and cared for on a daily basis were frequently discharged home while still in need of additional support, or not in a position to care for themselves.
With this trend likely to continue, Susan realized that there was a real need for professional in-home care services to help and assist patients through their care journey once discharged. These additional services would help ensure clients could maintain their independence and dignity while receiving the proper levels of care they need to keep them at home and out of the hospital.
