Keeping track of medicines and taking them at the right times is an important part of maintaining our health as we age. Many times, changes in a person’s condition or behavior are directly related to mismanagement of their medications. Anyone of us may get our medications or schedule confused, memory loss or loss of organizational skills can make this especially true.
Our Medication Management makes it easier for clients to set up and maintain their medication schedules:
Service includes:
Registered Nurses completing a reconciliation of all current medications, dosages, interactions and which doctor ordered it.
Finally, our nurse visits weekly or bi-weekly to prepare and pour medications into easily identifiable pill boxes for the client.
The nurse then coordinates medication orders with client’s current physicians.
If orders change or new medication is added or discontinued by the client’s physician, our nurse begins the reconciliation process again.
